Request Medical Records

Your referring and or primary care doctor should have copies of every note I have written. If not, your doctor's office may request a copy. Chart notes can be faxed free of charge to a legitimate physician fax number. It is no longer easy for us to get to your medical records and I may be out of town for weeks at a time.

As noted on the home page, THERE IS A FEE FOR MEDICAL RECORDS WITH PRE-PAYMENT REQUIRED - Specify chart notes only (just Dr Jasper's notes) or if you need the entire chart copied including financial agreements, permissions, telephone calls etc

We are permitted several weeks to respond. In order to fulfil your request we must first receive your request. Now that we do not have an office, we must first access the computer files, searching for any of several places your records may be stored, then count the pages and let you know what the fee will be including postage, sales tax and certified mail with return receipt options..
